Documentation

What documents are usually needed?

Requirements depend on your insurance plan. Here is a simple overview of what families are commonly asked for.

Comprehensive Diagnostic Evaluation (CDE)

A CDE is an evaluation completed by a qualified professional that documents a child's diagnosis. Most insurance plans require one before ABA services can be authorized.

Medical referral or physician order

Some insurance plans also ask for a referral or prescription for ABA from your child's doctor. We'll let you know if your plan requires one.

Insurance documentation

We'll ask for a copy of your insurance card and help you understand what your plan may cover. Coverage and approval are always determined by your insurer.

No CDE yet? We can help

If your child has not had a diagnostic evaluation, our team can explain how to request one and what to expect, so you know where to start.

Please don't send medical documents through our contact form. Our team will share a secure way to submit them. Eligibility, insurance approval and start dates can't be guaranteed and depend on each family's situation and plan.
